This Waiver and Release of Liability (“Waiver”) is entered into by and between the participant (“Participant”) and Gluteology, located at 1203 S Central Ave, Unit B, Glendale, CA 91204 (“Gym”), effective as of the date of acknowledgment.

Assumption of Risk
Participant understands that activities at the Gym—including but not limited to fitness classes, personal training sessions, and use of gym equipment—carry inherent risks, including physical injury, discomfort, or in rare cases, serious injury or death. By participating, Participant voluntarily assumes all associated risks, including those resulting in personal injury, property damage, or death.

Health and Fitness Acknowledgment
Participant affirms they are in good physical health and free from any condition that would prevent safe participation in exercise. They understand the importance of consulting a medical professional before beginning any fitness program, especially if they have existing health concerns.

Release of Liability
Participant releases and holds harmless Gluteology, its owners, employees, instructors, and agents from any and all claims, liabilities, or damages arising from participation in Gym activities, including any injury or loss, regardless of cause, including negligence.

Assumption of Responsibility
Participant agrees to follow all Gym rules and instructions provided by staff and accepts full responsibility for their conduct and safety while using Gluteology’s facilities and services.

Indemnification
Participant agrees to indemnify and hold harmless Gluteology and its representatives from any third-party claims related to their participation in Gym activities.

Media Release
Participant grants permission for Gluteology to use photographs, videos, or audio recordings taken during their participation for promotional use, including marketing materials, social media, and advertising. No compensation will be provided unless required by law.

Acknowledgment of Terms
By continuing to use Gluteology’s services, Participant confirms they have read, understood, and agree to the terms outlined in this Waiver and Release of Liability. This agreement is binding upon the Participant and their heirs, legal representatives, and assigns.

Governing Law
This Waiver is governed by the laws of the State of California.
